Youth Care Clubs and Health Connectors are two of the key youth differentiated care innovations of the Adolescent Innovations Project. YCCs provide group HIV management and care for young people including pre-packed ART medication and fun health-related activities, and screening for family planning and other health needs. Health Connectors link to HIV and HIV prevention care - through face-to-face meetings, mobile phones, and telephone conversations.
